KHCSA GATE RFID ACCESS TAG REQUEST FORM

NOTE: This form is to be used by KHCSA property owners to request a windshield RFID tag for direct entry into the neighborhood. Each tag request must be submitted through a separate form. The first 2 tags per property are issued free of charge to new residents. Additional stickers may be purchased for $15. NOTE THAT ALL REQUIRED FIELDS MUST BE COMPLETED BEFORE YOUR REQUEST WILL BE PROCESSED By submitting this form you agree to permanently install the issued tag on the assigned vehicle windshield following the guidelines included with the tag.
